How to Get Started
STEP 1: Contact the Regional Center
The primary step is to connect with your regional center to determine eligibility and initiate the process.

STEP 2: Assessment and IPP Development
A service coordinator will conduct an assessment and work with you to develop an Individual Program Plan (IPP).

STEP 3: Select a Provider
Once authorized, you can select Santa Cruz Supported Living (SCSLS) as your service provider.

STEP 4: Application
Fill out the necessary application forms, which may include details about your needs, background, and medical information.

STEP 5: Develop Support Plan
SCSLS will work with you to create a personalized support plan, including choosing staff and establishing routines
